The AppVeyor subscription has now ended, so latest builds are failing with timeouts and downloads of last-known-good are failing with download size limits.

The AppVeyor subscription has now ended, so latest builds are failing with timeouts and downloads of last-known-good are failing with download size limits. This change removes the AppVeyor run scripts and documentation references.
