Add files required for docker setup

This commit adds all files that are required to run formhub in docker.
You can build the docker image using

`docker build -t formhub:latest .`

And run it with

`docker run -d -p 8000:8000 --name formhub formhub:latest`

and finally enter the container with

`docker exec -ti <container id> bash`

This is useful when you "sign up" and want to set the `is_active`
boolean on the user account so that you can log in (as the database is
inside the container)
