you could add a newline after the fill-in spot so that "requests the following" is always on the line below, and the use that line to add the ‾ characters so that the fill-in spot actually has an unbroken line under it

the broken underlines don't look very good. You could add ‾ lines to fix this, or you could have have the fill-ins have no lines at all if you don't like the look of the empty space ‾ lines create. Either is fine by me, but the underscores are fugly
trying to make it look nicer, feedback encouraged
sorry if it looks amateur, I made it by putting together a bunch of existing assests from the base repository.
It's something to do as a chaplain, you can gather last wishes of the crew and arrange funerals appropriately
feel free to make any aesthetic changes as long and the base idea stays
