Location of database?

Where should the database be located and in what format? I tried using d/b file from Mac KeePass and it failed to authenticate, and I don't know where to look for logging. I placed it in the root directory of the install, and added
application:
db_file: 'PAAS.kdbx'

to the config.yml

I have been able to get my existing d/b opened by renaming it to "Testdb" and placing in the default location test/. After providing the main password, I see all my previous entries; however, the passwords are all missing (for the most part.)
