Add a customer feedback page

A page where customers can add their positive and negative feedback.

Add a check box on the page to request the customer/author of the comment whether it is OK for us (Subnero) to use the comments in our testimonial page and/or marketing collateral.

We'll need to figure out what platform we want to use for this. Currently, our website is completely static. For this, we'll need to have a mechanism for the user to send the information back to us. And also in what format it would reach us. Email? Slack message? Comment in our website?

Email should be simple enough I guess.

We should check if Zoho or Mailchimp etc have a mechanism to allow embeddable web forms for feedback. Then their platform will send the email to us.
