Prompt reason for override and log time spent

For given block sets or websites, it would be nice to ask the user a question "Why are you doing this override?" with a text field to provide a short reason.

It could also have a checkbox with a customizable text, such as "I attest that I need to use this website for an essential purpose or will be midful of the time I spend".

Then somewhere in the settings, it would be nice if it was viewable what those reasons were, along with dates and times, how long the override was for, and how long the user spent on said website(s).
