Remove cursors table #89

  • Remove the cursors table as it is not useful now.
  • Update module_hashes table to include the latest cursor for each (module_hash, chain)
  • Remove /cursors/missing query (or update)
  • Update /cursors/latest query (use module_hash instead of table)
