As an editor I would like a CC field when requesting major revisions by email

Problem

According to an OLH editor, there is still no CC field when sending an email to an author during the editorial decision "major revisions".

I believe this template is still using JQTE and not the new Django form.

Proposed solution

Refactor this view and template to use the unified Django form for template emails.
