Implement a multiple radio select with an "other" option

First of all, thanks for your project!

I'd like to implement/find a multiple radio select with an "other" option that allow users to enter their own answer. Something like that:

I'm affraid this link points to a code that is outdated (I'm using django 2.2)

Do you know such a module? If not, what approach would you take to implement it?
