Question

As I opened the setup file, and clicked Yes on the UAC dialog box, my antivirus software quarantined it. Is the setup file really malware?

Have you checked that no similar question(s) exist?

Installers are made by INNO Setup, so ask them if they have malicious codes.
The source code and the build process of DOSBox-X is all disclosed in this repository so that you can decide if it is actually malicious.

Not only the setup file, but you should also take a look of comments regarding security in using DOSBox-X software itself.
