Transactions Failing to Tokenize for New Customers - No Cache Enabled

Issue: Orders Failing to Tokenize/Charge
I've been experiencing an issue where orders are failing to tokenize and charge recently. Initially, I had caching plugins enabled, but I've since removed them for testing purposes.

Observations: This problem appears to occur primarily for new customers, but not every-time.
For existing customers, the issue has become less frequent since the caching plugins were removed a few days ago.

Error Message: The error message displayed to both customers on the front end and our staff on the back end is:
SecureSubmit payment failed. Gateway response message: “Transaction rejected because a token cannot be presented and requested in the same request.”
