Restyle Poll creation View

AC:
- Restyle "Title" text and field to look like "Name" in "Settings"
- Remove "Is active" Checkbox. All polls default to "not active"
- Under the "Is multiselect" checkbox there is a header titled "Options"
- Remove the "1. Option"/"2. Option" from left of the text field
- Instead autofill with greyed out "1." "2." "3."... text that shows what the response text displays when left empty, but isn't actual text in the box
- The "Update Poll"/"Create Poll" button is styled like the "Submit" button from the feedback page
- The "Update Poll"/"Create Poll" is right aligned, like the submit buttons for questions and feedback
- "Back" button is on the same level as the "Update Poll" button, left alligned

• Plz just make it look pretty, I'm bad at design
