`ExecutorCheckPerms_hook_installed` flag never set? #5

I see that the ExecutorCheckPerms_hook_installed flag is initially set to false, and then the check to install the enforcement mechanism https://github.com/hlinnaka/pg_quota/blob/master/enforcement.c#L32 tests against such flag. However, the flag is never set, seems like after https://github.com/hlinnaka/pg_quota/blob/master/enforcement.c#L35 the flag has to be set.
Or am I misreading this?
