associate a HS with an assessment

As an administrator I want to associate a HS with an assessment by finding healthsites within 500m of the assessment and associating the unique ID of the HS with the assessment

Notes:
Within the form of the assessment there needs to be a drop down with the unique ID's of the closest HS's
The administrator needs to be able to select the unique ID of the relevant HS
