The outline tab should be for the whole document and not only for the current Tex file.

Without parsing and expanding file inclusion, this is not possible.
As for #193 and #197 this would require to parse the LaTeX source, expand macros, etc… to be reliable and of any real use.

Yes. Regexp + recursion.
