Is it possible to handle a custom URL endpoint and run a command from an extension?

I want to add a custom URL endpoint (with URL path variables) like /show-item/{item-id}, and do some customized logic for the endpoint handler, which can be wrapped into a command from an extension.
Do you have any idea how to do this?
