Slack Hook URL Changed

The format is no longer this - https://SUBDOMAIN.slack.com/services/hooks/incoming-webhook?token=YOUR_TOKEN
It now looks like this - https://hooks.slack.com/services/xxxxxxxxx/xxxxxxxxxxx/xxxxxxxxxxxxxxxxxxxxxxxx - I really have no idea which part means what and so here's the link where I've found this type of hook url (with the old one I coudn't connect to any slack channels)
