You should be able to create new documents without creating them in the explorer tree. This is useful if you just wanbt to try a quick query etc. Once you have something you want to keep you should be able to save it by choosing a place in the explorer tree to put it. SaveAs should be replaced with a duplicate option so that you can quickly duplicate the document you have open without having to choose somewhere to save a copy.

As part of this change we should also force the creation of a unique document name on save.

StateDocStoreImpl, FeedStoreImpl need further work to ensure uniqueness of names.
