Alternative text for image and body_replace_img_at_bkm

First of all, thanks for that great package!

There is a problem using the alt variable in external_img() in combination with body_replace_img_at_bkm, the alternative text is not included in the Word document.

In combination with body_add_img all is correct, the alternative text is handed to the Word document.
