issue with github accounts with the same email

if two github accounts have the same primary email it seems when one account is registered the second account cannot register. i.e. the email holds some primary status. i'm filing this issue as a user has run into this.

• the original account is approved, but the user doesn't have credentials to use that one.
• the new account doesn't show up as a request for approval.

also i couldn't find a document that explains to an admin how to remove a user from the system.
