This seems to happen when there are two neurons that fire so frequently that there are many examples of near-coincidence events:
In this case they are particularly "non-local" but I'm not sure that is really the primary feature of them, they could just as well happen with two rapidly-firing neurons on the same channel - I saw that once and will post it here if I see it again.
