eCarePlanner 3.2.2 - Health Status Evaluation & Outcomes - integrate third-party endpoint data

eCarePlanner 3.2.2 at this time only queries the OHSU FHIR server for Observation resources.

Action items:

• Update eCarePlanner to query the SDS for all of a patient's Observation resources from each of their third-party endpoint partitions, and integrate these into the Health Status Evaluation & Outcomes tab.
• Due to real-estate constraints on this tab, it is suggested to add a "Source" dropdown select box in a prominent space somewhere at the top of this tab, so that a user can toggle around between data from different endpoints. The user won't be able to see data from different endpoints side-by-side, but that's a concession that we'll need to make, unless we want to design and implement a wildly different user interface for this screen, which isn't realistic.
