feat: cancelled any appointments screen

As a user, I want to state whether I cancelled any medical appointments to communicate my potential exposure level by visiting a medical office during a pandemic.

Acceptance Criteria

• It's done when I am prompted with a question asking whether I cancelled appointments.
• It's done when there's two options -- Yes or No -- and I can only select one.
• It's done when there's a back button available in case a user wants to change their previous answer.
