Symptom Questionnaire notification does not Disappear after completing it or populating in the heart health page

Description

Upon completing the symptom questionnaire, the survey does not disappear indicating that I have completed the task. It will allow me to go back in and input another response if i were to click the button again. The responses will also not populate in the heart health page on the app or in the generated health summary.

Reproduction

Happens for new patients that have just enrolled. Their symptom questionnaire responses do not populate into the heart health page and allow multiple submissions.

Expected behavior

The symptom questionnaire notification should go away after the patient takes the survey and their response should load into the heart health page on the app and in the health summary.
