Adding a feedback feature on each piece of content

@katiewilson21, the goal here is to have a form or some function that allows visitors to indicate if a piece of content was useful or not and/or submit/suggest updates to a piece of content.

Just to throw out an idea... on each page there are three smiley faces (happy, neutral, sad) and if you select neutral or sad, you get an option to submit written feedback.
