Document type is broken #249

In a vanilla substanced install, the Document type is broken; clicking it does not bring up the edit screen; the other two tabs show up fine.

Problem occurs in the first alpha release, both on Debian & OS X, Python2.7/3.4.

The scaffold was missing an include declaration, which caused the property sheet to never be added. Fixed on master.
