Schedule yes/no/maybe tick boxes and comments should be per item rather than in a great participant blob

I had comments on every item, and rather than commenting per item, they were in an amorphous lump that would require people to repeatedly check. It would be much better to have these fields by item so you could quickly accept the ones that were correct, and then make comments on the ones that weren't individually, so only the person running lead on that item needed to see those comments.
