Table caption and/or title are sometimes encoded as the first row of a table #110

Afraid I'm not going to get back to this until mid-June -- I'm out of

Example file attached. I'd be open to suggestions of where to add this
in meTypeset's current logic. This is a slightly lesser priority for our
corpus than #109 <#109>
