Table caption and/or title are sometimes encoded as the first row of a table #110

Some authors encode their table's titles into the tables themselves, and I don't believe that meTypeset currently touches table contents at all -- they're a direct conversion from the OOXML to the TEI to the NLM -- so they aren't detected.

I wonder if it would be worth trying to detect edit distance (Levenshtein or otherwise) between the first or last row of a table and all of the others, to see if one of these is likely to contain a title (would also require checking to see if only one column is filled in the first row so it doesn't instead contain thead, etc.)

Example file attached. I'd be open to suggestions of where to add this in meTypeset's current logic. This is a slightly lesser priority for our corpus than #109
