Dropping a spell from a Magic Item, using the Magic Items module, onto the Custom hotbar (when enabled for token hotbar) causes the spell to be placed in the same numbered slot on bar # 1 on the main macro bar instead, replacing whatever might have been there before.

This is most likely an issue of the custom hotbar - or the side effect of a third module (is there an error in the console log to identify the module?).
When dropping an entity onto the custom hotbar it monkey patches itself as main hotbar - and switches back to the real one. When something goes wrong and the switcheroo gets stuck half way through strange things happen.
I experienced this behaviour when an issues with Macro Folders raised an expection during deletion. Afterwards the drop behaviour was messed up.
