README docs do not work for ontology prefixes #18

Note: this is different than #17

the README has examples

    .prefixes.append(...)

I have found it to be necessary to make an ontology document wrapper and work at a lower level:

    doc = OntologyDocument(schema.CHR, o)
    doc.prefixDeclarations.append(Prefix('refseq', schema.REFSEQ))
    doc.prefixDeclarations.append(Prefix('insdc', schema.INSDC))
    doc.prefixDeclarations.append(Prefix('CHR', schema.CHR))
    doc.prefixDeclarations.append(Prefix('NCBITaxon', schema.NCBITAXON))
