Swap sam-text in modal docs for text input (#229)

* Updated sam ui elements and rebuilt dynamic files

* Changed to pull deploy url from variable

* Moved assets file path to use env var

* Added baseurl for federalist

* Added missing slash

* Replaced sam-text with input to fix modal issue
